While CDMA-only devices have never had a SIM, were there not Verizon CDMA/GSM/UMTS devices with SIMs prior to LTE? (e.g. iPhone 4s) These, of course, had GSM, UMTS and a SIM only for roaming.

Do not now nor have they ever used a SIM card. Only devices that support LTE support SIM cards on both Verizon and Sprint. Both Tmobile and At&t have used SIM cards since the GSM networks were introduced.

@DRNewcomb That is correct.
It should also be noted that even if the device supports LTE it may not support VoLTE which is required for continued service past the 2G/3G shutdown date.
Those devices were used designed to be used internationally only, as Verizon and sprint did not use SIM cards domestically until about 6-7 years ago I believe when They deployed LTE.
